Providing Assistance to the Transitioning Homeless
Eastern Georgia Hospice Veteran Partnership created Operation P.A.T.H. to help address one of the most overlooked challenges in our communities — providing compassionate end-of-life care and support for individuals experiencing homelessness.
While Eastern Georgia HVP focuses heavily on serving Veterans and their families, we often encounter homeless Veterans and vulnerable individuals who lack access to safe housing, healthcare resources, social support, and hospice care. Operation P.A.T.H. was developed as a long-term solution to help meet those needs with dignity, compassion, and community partnership.
What Operation P.A.T.H. Does
Operation P.A.T.H. is designed to create a mobile social model hospice and community support program that can travel where help is needed most. The goal is to provide compassionate care, temporary shelter support, and meaningful resources for individuals facing serious illness or end-of-life challenges while experiencing homelessness.
This program would:

Why This Program Matters
Homeless individuals often experience significantly higher rates of chronic illness, mental health challenges, addiction, disability, and lack of healthcare access. Many terminally ill individuals never receive hospice care simply because they do not have stable housing, caregivers, transportation, or financial resources.
Operation P.A.T.H. seeks to help bridge this gap by working collaboratively with healthcare providers, hospice agencies, social workers, volunteers, faith organizations, and community partners to create practical and compassionate solutions.
A Community-Based Approach
Operation P.A.T.H. believes real change happens through partnership. We work to identify unmet needs within communities and connect people with organizations willing to serve.
