About Eastern Georgia Hospice Veteran Partnership

At Eastern Georgia Hospice Veteran Partnership, our mission is rooted in one simple belief: no Veteran should ever feel forgotten, unsupported, or alone.

We are a community-driven partnership dedicated to connecting Veterans and their families with meaningful resources, compassionate support, and organizations committed to improving quality of life. Through collaboration with hospice providers, Veteran organizations, healthcare professionals, volunteers, and community partners, we work to ensure that every Veteran receives the dignity, recognition, and care they deserve.

Inspired by the national Hospice-Veteran Partnership (HVP) initiative, Eastern Georgia HVP helps bridge the gap between Veterans, community care providers, and available services. Our goal is to strengthen access to Veteran-centered care, increase awareness of benefits and resources, and create stronger partnerships throughout the communities we serve.

Beyond Veteran outreach, we are deeply committed to serving vulnerable populations within our local communities — including individuals experiencing homelessness. We recognize that many Veterans face challenges such as housing instability, isolation, mental health struggles, and lack of community support. Research continues to show that Veterans are disproportionately impacted by homelessness and often require coordinated community-based assistance to regain stability and hope.

• VA navigation and education

• Emotional and spiritual support

• Community partnerships and volunteer opportunities

Our organization believes that true service extends beyond healthcare. It means showing up for those who served our nation, supporting struggling members of our community, and building lasting relationships that create hope, healing, and dignity.

Through education, advocacy, outreach, and collaboration, Eastern Georgia Hospice Veteran Partnership continues to strengthen communities while honoring the sacrifices of America’s Veterans.
